2. Have the following things ready: Name, address, phone #, Social Security #, birthdate, insurance cards, employer address & phone if insurance is through employer or otherwise guarantor information including name address and Social Security#.
   

3. We suggest you contact your insurance company prior to calling for an appointment. You may need to get authorization first from your insurance company or they may have another provider site that you are required to go to for services.
   

4. For DCFS related cases, we need a DCFS form 431 consent. Please fax it to "ATTENTION: Intake"
